Membership Cancellation Form

We're sorry to see you go!

You will receive a copy of your cancellation request at the email address you provide.

If you do not receive a confirmation email, your request may not have been successfully submitted. Please contact our front desk right away to confirm.

Once your request has been submitted, please allow up to 2 business days for a member of our team to follow up and finalize your cancellation.

Cancellation Policy

Please submit a separate cancellation form for each student and each subscription you wish to cancel.

To cancel your membership, you must submit this online cancellation form at least two scheduled lessons in advance.

A lesson counts toward your cancellation notice only if it is more than 24 hours from the time your cancellation is processed.

Lessons canceled due to holidays or closures do not count toward your notice period.

Cancellations are processed Monday – Friday by 5:00 PM. Submissions after 5:00 PM or on weekends will be considered received the next business day.

If notice is given after the 1st of the month, no refunds will be issued for that month’s tuition.

Once billed, you may choose one of the following for any remaining lessons:

  • Take the lessons you’ve paid for
  • Keep a credit on file (never expires)
  • Gift your lessons to a friend, family member, or student in need

Stages Music Arts does not issue refunds for unused lessons.

A $30 registration fee will be required to re-enroll after cancellation.
